In accordance with the spirit of volunteerism and service, I, the undersigned, assume full responsibility for any injury or accident that may occur to me during my voluntary participation in the Business Meets Community Program activities.  I acknowledge that there are certain foreseeable and unforeseeable risks associated with participating in this event, including, but not limited to, injury, illness, traveling to and from the event, and the effects of the weather - all such risks being understood and accepted by me “Assumed Risks”.

Therefore, I hereby release, indemnify, and hold harmless United Way of Buffalo & Erie County, the organizers, the agency and project site at which I volunteer, as well as any sponsors and supervisors of all activities from all liability in connection with any Assumed Risks (including any injury caused by negligence), in conjunction with my volunteer activity.

I understand that I am responsible for complying with all COVID-related protocols in effect at the agency or project site, and understand that neither the agency nor the United Way of Buffalo and Erie County can be held responsible if I contract COVID or other illness.

I assign the rights for video, audio, and/or photographic recording(s) made of me participating in volunteer activity by United Way of Buffalo & Erie County or its agencies.  I authorize the editing, duplication, reproduction, copyright, exhibition, broadcast, and/or nonprofit use and distribution of these materials for purposes deemed suitable by United Way.  I waive any right to approve the finished products.
